Rarely Use Your Health Insurance? Do This Yearly Cost Math Before You Renew

When you rarely use care, the premium is almost your entire cost. A two-scenario comparison shows how much a richer plan is really costing you, and what you would risk by going leaner.

Scenario one: a normal year

Add twelve months of premium plus the small things you actually used: a few visits, a prescription, maybe urgent care. For a healthy person this total is usually dominated by premium. Preventive care like your annual physical and recommended screenings is covered at no cost on ACA plans, so do not count it.

Scenario two: a bad year

Add twelve months of premium plus the plan's out-of-pocket maximum. That is your worst case for covered in-network care. Leaner plans have a higher maximum, so the bad year costs more, but the normal year costs less. The question is how many normal years it takes for the savings to cover one bad year.

Making the decision

If the premium savings over two or three normal years exceed the difference in out-of-pocket maximum, the leaner plan usually wins for a healthy person, especially if the savings go into an HSA. If you have little savings, a lower out-of-pocket maximum may be worth paying for. Either way, compare the same doctors and hospitals across plans.

A real-world example

A healthy 35-year-old compares benchmark Silver ($673/month, about $8,076 a year) with Bronze (about $525/month, $6,300 a year). In a normal year Bronze saves about $1,776. If Bronze's out-of-pocket maximum is a few thousand dollars higher, two healthy years of savings cover the difference, and the HSA builds the cushion.

Your next steps

  1. Add up last year's premium and actual medical spending.
  2. For each 2027 plan, compute normal-year and bad-year totals.
  3. Count how many normal years cover one bad year.
  4. Check networks and prescriptions for each plan.
  5. Put the premium savings into an HSA or savings.

Questions people ask

What is the out-of-pocket maximum?

The most you pay in a year for covered in-network care, not counting premiums. After you reach it, the plan pays 100% of covered services.

Should healthy people ever choose Gold?

Sometimes, for example with a planned surgery or pregnancy, or a strong preference for low cost-sharing. For most low users, Bronze or Silver wins on total cost.

Does preventive care count toward the deductible?

Recommended preventive care is covered at no cost in network on ACA plans, so it does not need to.
