How underwriting works
You answer health questions, and sometimes the insurer checks prescription and medical databases. Healthy applicants may be offered a lower rate; people with conditions may be charged more, have conditions excluded or be declined. Because these products are not ACA-compliant individual major medical plans, they are not required to cover every essential health benefit and are not eligible for premium tax credits.
Where you find them
Underwritten coverage includes short-term medical insurance where state and federal rules allow it, Farm Bureau health plans in several states, some private health plans sold under different rules, and fixed-indemnity or hospital indemnity products, which pay set amounts rather than covering bills in full. Each category has different rules, so the label "private health insurance" alone tells you little.

Questions to ask before you buy
What exactly is excluded, including conditions you had before? Is there a waiting period? Are there caps on hospital, surgery or prescription benefits? Is the policy guaranteed renewable, and can the premium rise if you get sick? Is maternity covered? What happens if you need to move back to an ACA plan mid-year? Get the answers in writing.

Who this fits, and who should skip it
| Good fit if | Think twice if |
|---|---|
| You are in excellent health with no recent treatment | You have any ongoing condition |
| You pay full price for ACA coverage | You are planning a pregnancy |
| You accept some benefit limits for a lower premium | You need guaranteed renewability |
A real-world example
A 35-year-old with no conditions is offered an underwritten plan at well below the $673 national average benchmark Silver premium. It excludes maternity and caps some benefits. Because he and his partner plan to have children, he chooses an HSA Bronze ACA plan instead, but the underwritten plan would have fit a single person who accepted those limits.

| What you gain | What to weigh |
|---|---|
| Healthy applicants can pay less | Pre-existing conditions can be excluded |
| Some plans offer broad provider access | Benefits may be capped or limited |
| Can be bought outside Open Enrollment in many cases | Applicants can be declined |
| Premiums may beat full-price ACA plans for high earners | No premium tax credits and fewer consumer protections |

Questions people ask
Is underwritten health insurance legal?
Yes, in states that allow the specific product. It is regulated differently from ACA-compliant coverage.
Can I be dropped if I get sick?
It depends on the policy's renewability terms. Look for guaranteed renewable language and read how premiums can change.
Does underwritten insurance count as minimum essential coverage?
Generally not, though there is no longer a federal penalty. Some states have their own coverage requirements.
