When you shop for dental insurance, most options fall into two types: the dental PPO and the DHMO (dental health maintenance organization). They trade flexibility against cost in opposite directions.
How a dental PPO works
- You can see any dentist, but you pay less in network.
- Costs are usually a percentage of the bill, often following a 100/80/50 pattern for preventive, basic, and major care.
- There is typically an annual deductible and an annual maximum.
- Waiting periods are common for basic and major services.
How a DHMO works
- You choose a primary dentist from the plan's network.
- Out-of-network care is generally not covered.
- You pay fixed copays listed on a schedule — for example, a set amount for a filling or crown.
- Many DHMOs have no annual maximum, no deductible, and few waiting periods.
- Specialist care usually requires a referral.
Side-by-side
| Feature | Dental PPO | DHMO |
|---|---|---|
| Premium | Higher | Lower |
| Choice of dentist | Any; lower cost in network | Network only |
| How you pay | Percentage after deductible | Fixed copay schedule |
| Annual maximum | Usually yes | Often none |
| Waiting periods | Common | Often fewer |

Choose a PPO if
- You want to keep a dentist who is not in a DHMO network.
- You travel often or live in a rural area.
- You want the option of out-of-network specialists.
Choose a DHMO if
- Keeping premiums and out-of-pocket costs low matters most.
- You need major work and want to avoid an annual maximum.
- There is a DHMO dentist you like near you.
Before you pick
Look up the DHMO network in your ZIP code before comparing prices — a DHMO is only a bargain if good dentists participate near you. Compare how each plan handles the work you expect, and review annual maximums for PPOs.
